Virtual Enrollment can be completed by downloading the program's Enrollment application, submit the completed form, and required documents via email, and making the Application Payment
Each program's Enrollment Application can be found on our Home Page. Click the application to download to your computer , once downloaded there will be an Edit Button which allows the document to be completed. Complete the document, Save to your computer and submit via email. Send document to gwdmedicalacademy@gmail.com
Documents Required
A copy/photo of your I.D or Drivers license and High School Diploma or GED must be submitted via email.
Application Payments must be made via Program Payment Page, there will be time slots shown on this page these time slots are used to monitor the intake of applications . Once the application is processed a member of our team will contact to review the Enrollment Agreement and next steps!
