Online Enrollment can be completed by

Download Enrollment application AND Enrollment Agreement for selected Class
Submit the completed forms and required documents via email
Process Application Payment (Tuition Payment Page)

An Administrator will follow up 24-48 hours after process is complete to confirm Registration

Online Enrollment Steps

Complete Enrollment Application AND Agreement Form

Download the enrollment application and agreement form to your computer, once downloaded there will be an Edit Button which allows the document to be completed. Complete the document, Save to your computer and submit via email. Send document to gwdmedicalacademy@gmail.com

Documents Required

A copy/photo of your I.D or Drivers license and High School Diploma or GED must be submitted via email.

Application Payment

Application Payments must be made via Online Tuition Payment Page. Once the application is processed a member of our team will contact to review the Enrollment Agreement and next steps!
